    <dcterms:title><![CDATA[Surgical Management Of Cerebral Cavernous Malformations (Cavernomas) Causing Seizures In Children: The Cheo Experience. ]]></dcterms:title>

    <dcterms:abstract><![CDATA[Purpose: To review seizure outcome after surgery for cerebral cavernous malformation (CCM) in children presenting with seizures as the main symptom.<br />
Material and Method: We retrospectively reviewed the cases of 10 patients with cerebral cavernous malformations presenting with seizures as the main symptom. These 10 patients were treated surgically between 1988 and 2004, at our institution. There were 5 males and 5 females, age range from 1 yr 8 months to 1 5 yrs (mean 8 yrs).The clinical histories, and radiological findings, patholog y and operative reports were reviewed. Imaging studies were done asfollows: CT + Angiogram in 4 patients, CT + MRI + MRA in 3 patients, CT + MRI in 2 patients.<br />
The cavernomas were located as follows: temporal lobe: 5 cases; parietal lobe: 3 cases (2 in sensory- motor strip); and frontal lobe: 2 cases. All patients had diagnostic EEG, and vine (90 %) were on antiepileptic drugs. Five patients had partial complex seizures and fine generalized tonic- clonic seizures. Seven (70%) patients underwent tailored complete lesionectorny and epileptic focus resection as determined by the use of intraoperative electrocorticography(ECOG). One patient (10%) underwent complete lesionectomy and partialfocus resection. Two patients (20%) with lesions in the sensory motor cortex underwent complete lesionectomy only. Mean follow up time was 5 yrs (range 3 months - 20 yrs).<br />
Results: All patients tolerated surgery well without morbidity or mortality. All patients are seizurefree. Seven (100%) patients who unclerwent tailored complete lesionectomy and epileptic focus resection are seizure free with no antiepileptic medication. One patient who underwent lesionectomy and partial focus resection is seizure free on rnedication. The two patients who underwent lesionectomy only, are both seizure free, one patient still requiring antiepileptic medication.<br />
Conclusion: For cerebral cavernous malformations presenting with seizures in this age group, complete microsurgical excision is the treatment of choice. Surgery should be preceded by careful electroclinical, neuroimaging, anatomical and functional evaluation. The good seizure outcome results (100%) after surgical intervention in our patients are probably related to both the lesionectomy and the excision of the epileptogenic focus in non eloquent areas (80%). Nevertheless, the two patients with lesions located in eloquent cortex that underwent complete lesionectomy abone (20%), are also seizurefree. Based on thesefindings it would appear that complete lesionectomy is the main variable in determining seizure outcorne after surgeryfor CCMs presenting with seizures. Larger series will be required to statistically support this assumption.]]></dcterms:abstract>

    <dcterms:title><![CDATA[Changes In Ventricular And Intracranial Volume In Hydrocephalic Children Following Successful Endoscopic Third Ventriculostomy]]></dcterms:title>

    <dcterms:abstract><![CDATA[Objective: To investigate the change in the ratio of ventricular to intracranial volume following successful endoscopic third ventriculostomy (ETV) in hydrocephalic infants suffering from pure aqueduct stenosis.<br />
Materials and Methods: Using segmentation techniques, serial measurements of ventricular (VV) and intracranial volume (ICV) were performed on the preoperative and 12-month postoperative axial T-2 weighted MRI scans of 5 hydrocephalic infants who had successful ETV for aqueduct stenosis between 1999-2002. All patients remained asymptomatic, did not require shunting and demonstrated radiological evidence of stoma patency on Phase Contrast cine MR throughout the follow-up period extending from 2-4.5 years. There were 1 males and 4 females with a mean age at operation of 1.4 months (range 0.1 to 2 months). Each ventricular and intracranial volume measured was divided by the corresponding average normal volume for sex and age to calculate the &quot;x normal&quot; volume (xN). Each ventricular volume was divided by the intracranial volume to calculate a ventricular to intracranial volume ratio (VV/ ICV).<br />
Results: The mean ventricular volumes were: pre-operative: 295.3 cm3 (19.2 xN), at 12 months post-operative: 174.2 cm3 (10.9 xN). The mean W/ ICV ratios were: preoperative: 0.29, at 12 months post-operative: 0.14. For comparison, normal mean VV/ ICV was 0.018 for the first six years of life. All volumes and ratios pre and postoperatively were statistically higher than normal (p=.000).<br />
Conclusion: In response to endoscopic third ventriculostomy (ETV), ventricular volume and the ratio of ventricular to intracranial volume fall to values lower than pre-operatively but higher than the normalised values for age and sex. This further indicates that third ventriculostomy creates a state of compensated communicating hydrocephalus, with ventricles and head larger than normal.]]></dcterms:abstract>

Descripción. Paciente de 16 meses de edad con un cuadro de compresión medular y meningitis secundaria a un quiste dermoide intramedular. Estudios previos realizados al paciente no mostraron la presencia de una lesión expansiva intramedular Intervención. La cirugía permitió resecar en su totalidad un quiste dermoide abscedado intramedular.<br />
Conclusión. Los pacientes con estigmas cutáneos deben ser controlados periódicamente por la posible presencia, aparición tardía o progresión de lesiones intrarraquídeas. ]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[Objetivo e importancia. Si bien la hemisferectomía de Rasmussen es una técnica de probada eficacia, la reducción en el tiempo quirúrgico y en la pérdida sanguínea es crucial. Esto es logrado por medio de una variante técnica de desconexión en lugar de remoción del lóbulo temporal.<br />
Presentación clínica. Se presentan 21 niños operados entre 1991 y 2004 en quienes se efectuó una variante de la hemisferectomía funcional (HF).<br />
Intervención. La variante propuesta es una combinación de la hemisferectomía de Ras mussen y la hemisferotomía de Delalande efectuándose una desconexión y aislamiento del lóbulo temporal. Para ello, a nivel central<br />
se prolonga la incisión de la desconexión del lóbulo temporal hasta el piso de la fosa media la incisión de la cara medial del lóbulo occipital se extiende hacia el fornix para alcanzar el borde libre del tentorium. La unión del ambas incisiones desconecta el neocórtex del lóbulo temporal.<br />
Conclusión. Es opinión del autor que la hemisferectomía de Ras mussen es un procedimiento de probada eficacia. La variante técnica propuesta es particularmente útil para los neurocirujanos en formación en la cirugía de la epilepsia.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[Introducción. El vasospasmo post hemorragia subaracnoidea constituye una causa importante de morbilidad y mortalidad. Múltiples agentes han sido utilizados para su tratamiento, pero ninguno mostró resultados absolutos. El presente estudio se ha realizado para evaluar la efectividad del uso intracisternal de papaverina utilizado en forma profiláctica y terapéutica.<br />
Material y método. Se efectuó un análisis retrospectivo de los pacientes admitidos en nuestra Institución entre enero 2002 y julio 2003; 85 pacientes con hemorragia aneurismática de la circulación anterior y clipados quirúgicamente fueron divididos en 2 grupos. Grupo A: pacientes que recibieron papaverina a través de un reservorio y Grupo B pacientes que no recibieron papaverina. Se<br />
evaluó la localización, timing de la cirugía, incidencia y evolución post-vasospasmo en ambos grupos.<br />
Resultados. 13% de los pacientes del Grupo A y 38% del Grupo B desarrolaron vasospasmo. Los síntomas de vasospasmo revirtieron en todos los pacientes del Grupo A mientras que 11 pacientes del Grupo B con vasospasmo presentaron una significativa morbilidad.<br />
Conclusión. La utilización profiláctica terapéutica de papaverina intracisternal puede prevenir y revertir el vasospasmo, pudiendo ser inyectado a través de un reservorio.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[Introducción. En el año 1958, Smith, Moretz y Pritchard, derivaron el LCR<br />
(fluido cerebroespinal) a la vesícula biliar en 10 pacientes hídrocefálicos, las derivaciones fueron desde el espacio subaracnoideo lumbar en pacientes con hidrocefalia comunicante y desde el ventrículo lateral en aquellos con hidrocefalia obstructiva. La principal función de la vesícula biliar en la composición de le bilis, es absorber agua y electrolitos inorgánicos por tanto no tiene problemas en la absorción del LCR<br />
Material y métodos. Se realizó el proceder en 3 pacientes que fueron operados de Hidrocefalia en múltiples ocasiones y en los cuales no fue posible la derivación de LCR al peritoneo ni a las cavidades cardíacas. Se establece en los mismos una derivación ventrículo- vesícula biliar plenamente funcional. Se utilizó la técnica quirúrgica de Yarsagaray modificada por nosotros.<br />
Conclusión. Los 3 pacientes evolucionaron bien sin ninguna complicación y 2 de ellos llevan 3 años de operados sin disfunción de su sistema derivativo. Se demostró la eficacia de este método de tratamiento, dando una solución a la Hidrocefalia de estos pacientes y mostrando los mismos una evolución favorable.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[La lesión de la neurona motora superior en la parálisis cerebral infantil produce una perdida de la inhibición supraespinal sobre las motoneuronas del asta anterior de la médula espinal. La manifestación clínica predominante del síndrome de la neurona motora superior es la espasticidad . Esta espasticidad conduce a una discapacidad permanente, aunque no estable, para el movimiento. La toxina botulínica inhibe la contracción muscular previniendo la liberación de las vesículas con acetilcolina a nivel de los terminales presinápticos de la placa neuromuscular. La inyección intramuscular de toxina botulínica es un método eficaz y bien tolerado para el manejo de la espasticidad en pacientes con parálisis cerebral.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[Se presenta un caso inusual de un niño de 12 meses de edad que consulta por convulsiones debidas a hemorragia subaracnoidea secundaría al sangrado de un aneurisma de la circulacion anterior. Por un abordaje pterional derecho se clipa un aneurisma localizado en la unión de A 1 y A2 de la arteria cerebral anterior derecha. Una angiografia de control, efectuada 5 meses después, mostró un aneurisma &quot;de novo&quot; de la unión Al -A2 izquierda que se clipó a través de un abordaje pterional izquier do. La angiografia de control, 5 meses después, mostró una pérdida del flujo sanguíneo de la arteria cereral anterior izquierda, cuyo territorio pasa a ser irrigado por las arterias cerebrales posteriores. La angiografia de control 8 meses después, mostró el patrón angiográfico antes descripto sin nuevos aneurismas. El niño evolucionó sin déficit neurológico alguno.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[La separación de la duramadre del cerebro subyacente en las reoperaciones, habitualmente se hace mediante coagulación bipolar, corte de vasos y adherencias y disección roma. El conocimiento de la forma de trabajar de la aspiración ultrasónica (Caviton Ultrasonic Surgical aspirator CUSA) llevó a los autores a utilizarlo en la interface cerebro-duramadre, para reducir el sangrado y la agresión al tejido cerebral, posibilitando la apertura dural en las reoperaciones.<br />
Se examina el efecto de esta técnica en las reoperaciones de 10 pacientes operados previamente por padecer<br />
epilepsia (5 temporales y 5 extratemporales). Rápidamente se logró despegar la duramadre del cerebro, permitiendo visualizar la superficie cerebral sin distorsiones. La técnica consiste en la aplicación del aspirador ultrasónico en el ángulo diedro formado por la duramadre y el cerebro, lo que produce una inmediata desvascularización de la duramadre, reduciendo el sangrado no sólo dural sino también del cerebro.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[Objetivo. Presentar los resultados del tratamiento endoscópico de los quistes aracnoideos de la fosa posterior. <br />
Material y método. Se revisan las historias de pacientes tratados entre los años 2000 y 2004 evaluando edad, sexo, hallazgos radiológicos y tratamiento previo. En todos los procedimientos se utilizó un endoscopio rígido. <br />
Resultados. Encontramos 6 niños y 3 niñas (edad promedio 29,9 meses) La fenestración endoscópica de los quistes fue útil en 7 de los 9 pacientes. De los 4 pacientes a los que se había implantado válvulas previamente, 3 no requirieron derivación.<br />
Conclusión. El tratamiento endoscópico de los quistes aracnoideos de la fosa posterior debe ser el de primera opción en las instituciones que hagan esta práctica de rutina.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[Objetivo. El estudio de las manifestaciones clínicas y evaluar las diferentes modalidades de tratamiento quirúrgico y sus resultados en todos los casos.<br />
Material y método. Se realizó una revisión crítica y sistemática de la literatura en base a los criterios de la medicina basada en la evidencia (MBE). el método implicó: estrategia de búsqueda bibliográfica, criterios de cribado y exclusión de estudios, análisis y síntesis de los resultados relevantes de los artículos seleccionados. La revisión se realizó a partir de 1983 a 2003, utilizándose la base MEDLINE, en términos de MeSH (Medical Subject Healing). Se seleccionaron 17 de 77 artículos.<br />
Resultados. De los 18 artículos revisados identificamos 7 con población pediátrica, cinco del grupo C y dos del grupo D, se incluyeron un total de 108 pacientes, con siringomielia asociada en el 27.7% y el principal síntoma fue la cefalea 21.78%; de los artículos con población mixta fueron 11, cinco del grupo B, tres del grupo C y 2 del grupo D, se incluyeron 614 pacientes, con siringomielia asociada en 53.74% y el principal síntoma fue la alteración sensitiva en 47.01%. El principal procedimiento quirúrgico realizado fue la craniectomía suboccipital 86.8% y 95.5% respectivamente, seguido por laminectomía 76.19% y 55.5%, y duroplastía en 62.85% y 33%. Durante la evaluación postoperatoria con una mejoría clínica del 77 al 100% en los artículos de población pediátrica y del 80 a 97.2% en los de población mixta.]]></dcterms:abstract>

    <dcterms:abstract><![CDATA[Introduction: At present with the globalize world there is substantial information of medicine cost analysis. In neurosurgery main expenses comes from diagnostic studies, medical supplies, drugs and the neurosurgeon&#039;s fees. In this paper 1 will try to examine and compare the costs variables of neurosurgery between Latin America and U.S.A. <br />
Method: The comparison is done between the costs at a University Clinical Hospital in Latín America with a standard University Hospital in U.S.A.<br />
Results: While the price of the MRI, CT, angiography equipment and other instruments are about the same in both areas, the price for these exams may be 6 to 1 O times higher in U.S.A. Investment for research at the universities may increase these costs. The neurosurgeons fees for a mayor neurosurgical intervention (brain tumor, epilepsy surgery, etc), is 5 to 10 times higher in U.S.A. than in a Latin America country like Argentina or Chile.<br />
Conclusion: From this comparative analysis, U.S.A. neurosurgeon&#039;s income is several times higher than their Latín American colleagues. However their taxes and malpractice insurance costs are also elevated. The latter in Latin America is increasing but at a mild pace in most countries. In conclusion the main difference on the cost of neurosurgery in Latín America and U.S.A. Comes from the neurosurgeons salaries and secondly from the infra-structure. With globalization it may be possible that in the future the North American private health insurance companies may pay theirs patient&#039;s trips to be operated in selected centers in the South.]]></dcterms:abstract>
